How thumbnails pictures are generated?

Hey hey,
What are the rules behind how/when thumbnails are generated?
I've encountered that some links/webpages have nice thumbnails while other haven't at all.

Thank you. That's explain a lot.

Do you consider add manual upload of image in case there is no picture at all? Maybe I'm in minority but I prefer to have some "visual" context for each link.

This was requested in #301 already, but I personally don't need custom uploaded thumbnails, so I probably won't work on it.
