Fixes #1001

Summary

Adds a single unified GitHub Issue Form for bugs/features/docs/questions/proposals.

Notes

I did not change .github/ISSUE_TEMPLATE/config.yml to avoid altering existing issue flows (blank issues/contact links) unless requested.

Test plan

Open “New issue” and confirm the “General issue” form appears and fields render correctly.
